Documented risk register, JSAs at task level, and permit-to-work for high-risk activities, hot work, confined space, live electrical, refrigerant.
Tickets tracked centrally. PEW registration, gasfitting, refrigerant handling, working at heights, EWP, confined space, all current, all visible.
Tools are tested, tagged and replaced on schedule. Fleet inspected and maintained on a calendar, not on a "when it breaks" basis.
Every incident and every near-miss is reported, investigated and closed out. Patterns roll up to monthly senior reviews.

Nobody handles a piece of plant they're not ticketed for. Apprentices work supervised. Tickets are tracked centrally and renewed before they expire, not after.
